Aetna Worries

My surgery is set for Dec 14th, or so I thought. I received a letter from Aetna today that lists the service dates for the procedure as 3/16/17 - 3/16/17.

Is this the deadline for the surgery date or the date they expect me to have surgery? Notice the admission date at the top left is also 3/16/17.

Hope someone can help.

The doctor's office had not sent in the final surgery date. (My doc has rearranged his staff, and this is not the first time the new nurse has dropped the ball.) All is well now, and surgery date is still Dec. 14th.
