Approval for how long?!

So I got the call today that my insurance approved my surgery!!

Yes!

On to my question, I am a smoker and my surgeon requires his patients to be 60 day nicotine free before he'll perform any procedure. Fine, no big deal. I should already have been quitting to anticipate the procedure.

My question is how long do surgical approvals last? I'm courting the idea of February for my sleeve but I'm curious as to whether I'll have to seek another approval or if once it's approved I can do it whenever?! The insurance company and plan will not be changing in 2017.

Any advice is appreciated!
