BCBS FL How to get approval

Hi guys! I'm new here so I'm sorry if this is the wrong board or whatever, but I have a question.

My insurance is BCBSFL and while researching, I learned that BCBS does not cover bariatric surgery in Florida. But I do have a BMI in excess of 40, and I was wondering if there was something I could do like submit a form or something that would get them to cover it? Has anyone had any luck or experience with this? Thanks in advance. :)

I have BCBS Anthem through my employer. I emailed them and asked if it was covered and the requirements. I received back the detailed information and worked with my surgeon's office. The surgeon's team can be helpful with finding this out too.
