I completed the criteria and Anthem Blue Cross has denied me because of the 6 months weight loss criteria. I went to my PCP for that and I have no clear information on what I need to do. Any ideas?

I have them too. Your pcp should send all of the documentation to the surgeon to be included in the packet they submit. Did they do this? What does your surgeon say?

The surgeon is calling then for a peer to peer on this. Yes my PCP sent over everything and still they are not clear on what kind of documentation is needed.

I have anthem too (Indiana) I will have completed my 6 month supervised diet coming this Friday, I saw the NUT and Dr every month for 6 months and they have documented all my weight loss during that time, and Monday the Dr Office will submit my paperwork to Anthem, I've already completed the psych evaluation, upper GI, Doctors recommendation and have Diabetes. I hope to have an answer within a couple of weeks after submission. DID they send all the documentation of your 6 months of visits?

Yep. If u get approved maybe some details on what is need on it paperwork would be helpful.
