Net gain?

I have Aetna insurance PPO II. In the requirements it says that there must not be a net gain. What exactly does that mean? Like no weight gain at all? Or you can't be more than the first weigh in?

I have Aetna in NJ. My 1st visit with the NUT 233.9lbs, 2nd visit 235.0, 3rd visit 233.5 4th visit 233.0. BMI 40-42. Coordinator submitted to Aetna 3/14. Approved for band to sleeve revision on 4/12. Surgery is scheduled for 6/7.

Weigh yourself weekly and replace meals with Protein shakes when you can it will help get ready for post op. You absolutely cannot weigh more at the last NuT visit than the first visit. Aetna is very strict.
