Have you been weighing in? You need to have 3 recorded monthly weigh-ins over 89 days or more. I did mine at my Bariatric clinic. My clinic required me to lose weight, Cigna didn't care if I lost or gained.

Cigna also requires a letter from your PCP recommending Bariatric surgery and a psych eval.

Aside from the BMI requirements, I think that's all Cigna needs.

I was approved as soon as they received the request. No problems at all and they were really good about approving the classes and everything pre-op and the sleep study, too.
