Bmi is 36.88 with few problems high blood pressure, high sugar cholesterol high and high risk for heart disease doctor filled out my medical necessary for for a good candidate. I have uhc r they hard to get approval meet with sugeon he set my date for 4-5-16 . They said they submit it 2 weeks before my date. Any advise would b helpfull. Thanks
