Lots of posts about Cigna if you do a search. I have them and my surgery was covered; however, the customer service told me about 50% of their plans do and 50% don't. It depends on the employer / purchaser of the insurance and whether or not they specifically excluded bariatric surgery. If your specific plan does cover it, then it is 3 months pre-op diet requirement, letter from Primary Care doctor (or doctor other than surgeon), over 40 BMI or over 35 BMI with co-morbidities.
