My insurance told me yesterday the request was for Oct 19. My dr didnt tell me that. Should I tell my job?

Call your doctor's office to confirm the surgery date before telling your job. Usually the surgeon's office picks a random date to submit but will give you a different actual date for surgery.

They have to give the insurance something and once approved can change it easily

It didnt matter they are denying me because of the 6 month nutrition requirement

FYI - you do not have to tell your job. When you do get approved, the part of HR that knows cannot divulge due to HIPAA. When put in for leave to time off you only have to state you are taking a medical leave. If you take a week or less off, I would just use vacation time. However do not put in for leave until your surgery date is 100% certain.

If you have not finished the 6 months yet - are you at 5? Otherwise why are they bothering ? Missing a lot of details here

No I haven't finished yet. The original post was prior to denial. I submitted other dr notes for requirement. My ins sucks. I am nervous they will deny for different reason once I complete their requirement
