There are several reasons this could happen:

1) Heart cannot be cleared for surgery to happen safely (anaesthesia concerns)

2) Person is unable to commit to changes that will be required for after the surgery.

3) Psych eval does not indicate that patient is a good candidate.

4) Severe adhesions/scarring make surgery too high a risk.

5) Patient will not stop smoking making surgery a risk due to higher incidence of a bleed, etc.

Not sure why you are asking, but there are some very real reasons a person may not be a good candidate.
