Body measurements

This is going to sound like a really dumb question but.... oh well

When measuring the arms are you supposed to pull the tape measure taunt? Or do you just loosely encircle the arms?

I ask because my arms have alot of sagging and it seems like the measurement hasn't changed, even tho it seems like my arms have gotten smaller.
