Losing/Gaining before surgery

I am a pre-op sleever dreamer. I am almost done with my 3 months of supervised NUT visits (as that's my insurance requirements). At every visit so far, I have lost weight. I have not tried consistently to lose weight, I am just trying to eat better-exercise more.

My question is, has anyone had any problems with insurance declining due to weight gain or not enough weight lost during the NUT phase of the pre-op requirements?
