Weight gain:( need help getting back on track!'!

I had gastric sleeve surgery April 2013 before that I had lap band surgery put in and takin out...I weighted 205 at surgery my lowest weight after my second surgery was 147 now I'm back up to 160...I need help getting back on track!!!
