Finally the case manager at the doctor office faxed me over the completed paper work! Only waited 10 months smh.... I rushed n faxed it over to my insurance company n surgeon office. My insurance company said 15 business days to get a answer!

Hopefully it will be sooner.. Gonna be staring at the calendar now lol. Wish me luck. Hopefully to have a October date!
