Hi are you terrified of the unknown or success? We are all afraid of the unknown but you can not let your fear overcome what you know is the right thing to do, if you have tried everything as we all have, then you know that your goal is to be healthy, we all ready what unhealthy feels like. Good luck.

Talk to your surgeon. It's common to have anxiety before a surgery, especially if you haven't had one before. He can talk you through things and may offer to give you something to help with the anxiety -- help you get some sleep.
