    Federal BC/BS, and they might be different. Took six months to get through the medical and educational requirements. My out of pocket costs were around $1,000 whaen all was said and done. Find there web page and research their bariatric surgery coverage!

    They have no info online about bariatric coverage but I have several emails from customer service saying they do cover it
