HELP!

My insurance asked for 6 months info before I could send in paperwork for approval.. yesterday was my last appt I faced over my paperwork and they lay at the surgeon office tells me oh I just found out you need 6 months exercise log.. I'm so mad I even asked her 6 months ago if that is required and I was told no so now that I have done everything else this comes up on the last day before I should submit my paperwork. Idk what to do. I need help do I just make something up or do I just not do Idk? Has anyone else have to do an exercise log that can give me an idea of what insurance is looking for?

Do you use a gym? If yes, they should be able to print out a log of when you checked in

Ask your coordinator what is a standard exercise log. They may just need an accounting of what you did.

You could set up a calendar sheet on the computer and fill in to the best approximation what you did, walking, jogging. . I can't imagine they would require you to be a member of a gym or specific program without letting you know that in advance.
