Just wondering , i have to ask my doc to write a verification letter to my insurance for lapban,how do i go about asking my doc?shouldent i have form or somethin for her to fill out?

Your Dr.'s office should have all the info they need for the letter from your file. Some offices charge for filling out any forms---others fill them out, write letters, whatever, as part of your care. My PCP, wrote several letters for me, all at no charge. He always made sure I got a copy of anything he sent. I will say after the first one, I truly wished he would let me write it and him sign it---he was short and concise, I felt he should have detailed more----but it is in the past now!!!

I just asked my doctor to write me a recommendation letter and he did, free of charge. I gave him an idea of what they were looking for and he put it all in the letter and gave me a copy. It did the job cause not long afterwards, I was approved!
