BcBs florida anyone get approved?

I'm getting very nervous as the time to submit draws closer. Wednesday I have my last visit which includes the liver class. My bmi is 36 with Co morbid of high blood pressure diagnosised in June this year, gerd, and rem sleep apnea. Did anyone not have to submit pass weight history for BCBS? I'm on the state employees plan and they offer an awesome contract. What was your bmi& comorbids?

In Florida they require 6 months consecutive of documented physicians records for weight loss. It's a PITA

Anyone work for BCBS, or anyone have bcbsfl and know their requirements for wls? Are state and federal policies similar?

My coordinator said within 2 years. Not necessarily the 6 months before you submit to insurance.
