bcbs approval without 5yr documentation?

My policy states for approval you must have a bmi greater than 35 for 5+ years. I have no documentation I can prrovide that proves this. Will that be a problem?

Don't you have a regular Dr. that you'd go see. They should have an intake form with your weight for ea. visit, which they can then figure out your bmi from. I had to call 2 different Dr's offices for my intake forms, and then faxed them to the surgeon's office. I was approved within 48 hrs.

I was in my 20s and just went to an urgent care when I needed to see a dr. Ive since moved and that urgent care is no longer in business :-(

I agree. That you can you're the with from your doctors visit I also had WW send me a copy of my weight and any physician supervised diet will have that info.
