  14. I have Anthem and have called them many times to find out . Each time im told they don't have a time for a supervised diet. My company changed to them January, I started seeing the nutritionist in November. The office says it takes 3 months. I found info online that says they require 6 months supervised diet. It says they will allow if it was done within 2 years of the request. I had a failed attempt last year. I had my last visit to the nutritionist on the 28th. The office is going to submit. I will patiently wait for the response.
