Anthem - new jersey

Anybody have Anthem in Nj, I wanted to know their requirements. I know everything but the nutrition. The policy is not clear, when I called they said there is no specific amount of time. The office coordinator says 3 but im not sure. I did 3 starting in November. I also did 6 months a year and a half ago. Does anyone know if they will accept this.

All my info has been submitted but im afraid to call the insurance. I think they will tell me im denied.
