First Pre-Op Doctors Visit

Hi everyone!

I am new to the site and have just recently started researching the LAP-BAND®. I have an appointment with my PCP on October 8th and am preparing some information and questions to take with me.

So far, I have the following:

- General history of weight issues

- Weight loss attempts (including methods and timelines)

- Family background/indicators

- Co-morbidities

- My personal reasons

- My support system/living situation

I also have lots of questions for my doctor that I'll be taking with me. I have spoken to my insurance (HealthNet) and am told that coverage and any pre-op requirements depend upon my medical group and pcp.

I want to be as prepared as possible in this meeting! I love my pcp and have great respect for her. I am curious if I'm missing anything. Your help would be GREATLY appreciated!
