Dr. Christopher Evanson - Banded by Dr. Evanson

I was banded by Dr. Evanson 8/4/2009. I had my first fill 9/15/2009. Anybody else out there banded by Dr. Evanson? Or at St. Vincent's Carmel Hospital.
