Fills happen after surgery. After you've healed for a certain length of time, every surgeon is different, you will then go to your surgeon's office for a fill of usually saline, that is inserted into the port that is just underneath your skin. The port is accessed by a needle filled with the saline. The port is attached to a tube that is attached to your band. The band had little "pillows" along the inside diameter of it and the amount of saline in the "pillows" is what causes restriction on the stomach. This causes you to eat less at any one sitting and feel full faster when you have proper restriction.

There is a whole section on here about the band and if I remember right, even some pictures. If you do a search for lapband, you will also get the manufacturers' websites and they have detailed pictures of the band, tubing and port on their sites.
