Bradley Schwack MD

Does anyone know the whereabouts of Dr. Bradley Schwack? He was associated with NYU Langone in NYC. He left that practice, but even after two years, he's nowhere to be found. His online presence still shows him to be with NYU Langone on First Avenue in NYC.
