Anthem BCBS

I know all the plans are a bit different but if you have Anthem BCBS, can you tell me about the process for you?

My BMI is 40 and I've always had mild sleep apnea. I'm sure they'll do blood work etc when I have my consult on 12/14.

I'm hoping to convert from Lap Band to Bypass.

    Call your insurance company. Confirm your coverages for Bariatric surgery.

    Then ask the for their policy, in writing, to be sent to you.

    Most insurance companies have written policy bulletins that describe the qualifying process.
