Bcbs Michigan

Hello, I just finished my 6 month visits. We have BCBS Michigan. My dr office submitted my paperwork last week. I just noticed (basic online Bcbs Michigan Google

Search) that they also require proof of

12 months of failed dieting NOT documented by surgeon. This wasn't the case when I started the process. Anyone else experienced this?
