Pre-Op Practice?

Hi - I am new here. Just got my phone call from the surgeon's office telling me it's time to set up the pre-op food plan for the next 6 months. For those who had to do this 6-month pre-op, if you happen to lose significant weight during that time - does your insurance then deny you the surgery? Or is there a basic formula for determining how much you should maintain or lose during that time period?
