BCBS of Minnesota

Okay so in 2013 I was going to get surgery through UHC and completed all the requirements, 6 month supervised diet, psych evaluation, meeting with the surgeon. Even had a surgery date. I was laid off and lost my insurance though and was not able to get the operation. Now in 2016 I have BCBS of Minnesota and I'm wondering will I have to do it all again? Any info about your experiences are greatly appreciated!!!
