I seen the surgeon 2 weeks ago and am waiting approval now. I have a tentative date for 4/27. My surgeon had me fill out a 6 month weight history and have my Primary doctor sign and stamp it and also I seen psych and nutritionist the same day. All that was done in one day. I just have to see cardiology and pulmonary for pre op clearance but that has nothing to do with auth
