How is payment made?

I am new to this and have my referral letter. I'm not rolling in money and wanted to know how the payment works?

Do you pay the gap only and the clinic chase the rest?

Do you have to pay the whole amount and then do the claim back process?

I have top cover hospital with HCF.

If I go ahead with the operation money will help my decision.
