I do not know if it is required, but I have an appt with my primary doctor to state that I feel I need WLS. I am hoping to obtain a letter of support from him. I will be bringing my history and proof of payment for various weight loss attemps as well as my last years blood test results. I am going to ask for a new weight and a height (shoes off this time). I have no idea of his opionion of WLS but I thought it wouldn't hurt to try for a written recommendation. I also will have him look at my chart. I had a BMI of 38 two years ago and then I quit smoking. I now have a BMI of 42. (But my lungs are now healthy:)).
