The fill is how the band works.

The band is a silastic balloon-ring that goes around the stomach and restriction is achieved by inflating (or filling) that ring using a needle into the port that is placed in your abdomen.

Here's a picture of the band with the port attached, the port is sutured to the fascia of your abdominal muscle.
