does anyone have Prominence *** insurance plan?

i was surprised to find out that this insurance covers bariatric procedures! it may end up costing me $2500 instead of 8-15k so i'm excited about that. does anyone else have experience with this insurance? do you know what their pre-op requirements are to get approved? i don't want to do this 6 months to a year of dieting. that seems ridiculous. i've been preparing all my life! shit in six months i could be under 35bmi and then i'm screwed and they won't do the surgery and i'll just gain it back like i always do! help!
