Missing office note for 2013

I have BCBS of AL which requires an office visit note from 3 years prior to surgery showing height, weight and BMI for Morbid Obesity. I have 2012, 2014 and 2015 but no visits in 2013. Seriously?? :(

Anyone have this problem and get approved?

Most of those things require a formal documentation of your height and weight and Urgent Care/ ER/ or any form of medical visit should be able to be used in place of a note from your primary care doctor. As long as you have some type of proof, it seems like it would not be automatically declined.
