I have HighMark BCBS of PA as well and besides the 6 months visits I didn't have the only other thing required is whatever the doctor requirements were and mine consisted of Cardiologist, PCP, NUT, and pysch eval I received my denial in two weeks would have been an approval had I known I needed the 6 months eval, however my coworker had the same insurance last year and she did not have to complete the 6 month visits perhaps it changed so good luck to you
