    I was wondering if I have to call insurance company myself to se if I am approved I called before initial vist with codes to see if they were coveredand they were and when I went for first visit they told me requirements my insurance has I had to lose 15 pound have a psych eval and go for testing and blood work and see nutritionist 6 times in 6 months
