How do you make a **poll**

I want to have a poll on pain after surgery.

1. hospital stay vs out patient (NO COMPLICATIONS)

2. what you were given for at home pain

3. pain after surgery for how many days

4. degree of pain...10 being your arm was cut off and 1 being a paper cut.

5. Doctor and state where surgery was done.

I need help doing this...since I'm computer challenged.:help:
