Insurance Approval

I've done almost all preop testing. Still need a stress test and echo of heart due to a stent placed in 2005. Also have enlarged liver/spleen due to high triglycerides (I'm diabetic). Has anyone met with difficulty getting insurance approval from a Medicare Advantage Plan (Today's Options)? I'm

51 and disabled so on Medicare. I have many comorbidities. I just want this surgery done to get my life back on track. I've lost 25# while planning for it.
