My Pre-Op Schedule!

Step #1 Weigh in Visit at Florida Surgical Weight Loss Centers, on 11/26/13 Time 9:30 1. Please come to the office so you can be weighed in for the results of your liquid diet (had to lose 10 lbs.). 2. You will then sign your consents for surgery. Step#2 Pre-Op Registration at Hospital Date 11/26/13 Time 11:00 1. “Please be fasting for 6 hours” 2. “Patient should report to the E.R Dept. of Memorial Hospital for registration and then will be directed to the pre-op dept. for testing” Step #3 Surgery Schedule Date 12/05/13 1. You will be instructed what time you should report to the hospital on the day of you surgery during your pre-op evaluation at the hospital.
