It all depends on your doctor and insurance company and what each requires. I went for a class at the end of January, had a consult. visit schedule a couple days after that and that is where my doctor put a plan together of tests, nutrition classes and evaluations, and requirements needed for insurance submission. My surgery was April 9th so it was like 2 months for me. There are a lot of people that have to be on a 6 month supervised diet. I was lucky where my insurance did not require that. All doctors and insurance are different.

Hi I am brand new to this and I have Horizon too. Just found out that I have to do a 6 month diet program, been obese for at least 5 years, have 2 letters of recommendation, and a BMI over 40, etc. I am about to schedule an appt. w/my family DR for the 6 month physician assisted diet. Have you learned anything you can share?
