There really isn't anything to be prepared to answer. There don't seem to be any standard questions. Just be relaxed and honest.

Well I'm back from my visit. I had to answer true or false to about a 400 questionnaire. Then she asked me was I suicidal or homicidal. Do I hear voices in my head. Do I feel like I'm being controlled. What are my health issues. My current weight. What diets have I ever attempted. What have I done to prepare my self for surgery. Do I have a support group. What do I eat for breakfast lunch and dinner. Where do I work. Who lives in the household. She asked a ton of questions. Glad it's over. Hopefully everything I said clears me to be okay. That questionnaire was a pain...
