I got one from Dr. Sanchez after surgery. It's very handy to give to Doctor's and hospitals to explained that you have a band if for some reason you weren't able to tell them yourself or they see something odd in a test or something. I keep my in my wallet and every physician or hospital I go to I give them a copy.

I got one and I keep it behind my driver's licsense. It just has a pic of the band on a stomach and it has my surgeons name, number, and the fact that i have a lapband... that kind of thing. I got mine in a packet of stuff from Inamed that my surgeon gave me on my first office visit after my surgery.

I got one from my doctor at my first follow up visit. I keep it in in my wallet. I have ordered a medic alert bracelet that states brief info about my band and that there is more info in wallet.

I dont have one. Never heard of it, heard of the food card but thought it was something you printed out and had your Dr sign for discounted food.
