Sleeve Coverage -Bcbs

I have bcbs ca. Do most all bcbs plans cover the sleeve? Does anyone have the medical code for the sleeve procedure? I guess I just assumed it was covered, but now I'm getting a little nervous.

  • So I found out today from my surgeon 's pa that the sleeve is covered. Yeah! I'm just waiting to set up my pre op tests & final approval.

I did find out that the sleeve is covered Yeah! My requirements were a three month program: 1 weight loss class, 1 nut visit, and a psych test. My surgeon also asked that I lose 20lbs. I've met all requirements as of yesterday. My paperwork will be submitted by the end of the week, and I should be hearing from the hospital to soon for my pre op tests.

Those who have had the surgery, if you don't mind me asking, did you have to meet your deductible only or your out of pocket and deductible before the insurance kicked in?

BCBS CA here as well. What were the insurance requirements? I found out I'm covered (I am the employer and I guess got lucky that I picked the right plan that covers VST!).

I am a lapband revision, but I'm concerned that I would qualify for a new procedure (33 BMI, etc.).

My Insurance requirements were a single nutrition visit and a referral from my PCP. I have the access plus BCBS plan.
