Help!

I really could use a mentor. I'm a 31 year old female. Going to the doctor's seminar tonight. I have Anthem and was told the procedure is excluded and I have to write a letter to the Pre-determination department. I live in Southwest Missouri. Any help is appreciated. Thank you!
