No one but the insurance company is going to be able to answer that for you. Call them or look it up on their website.

Have you looked up your BMI, is it over 40? With Tri-Care you have to have a BMI 40 and over, or 35 (I believe) with co-morbitities, go to the Tri-care website and type in Bariatric Surgery, and the list of requirements will come up...GOOD LUCK!
