First Dr. Appt. Tommorrow...help!

I finally have my first appt with the dr. tommorrow and I have no idea what to ask! I always go in with questions, then get so nervous I forget them!!! I am trying to fix this by making a list of questions to ask, but I am stuck. Can someone please help me!!! Thanks.

I actually have to do a 6 month diet for insurance and I have only had 1 appt. So at least 5 more months before I can even think about it. But I am sooo ready to have the surgery. Thanks for the questions, I hadn't even wrote anything like that down! I am so thankful for your help!!!

Cost of fills?

Protocol after a fill?

Is Dr available by phone if you have an emergency or very painful stuck episode?

How many CC's does the Dr put in at surgery?

How strict is your Dr regarding diet and nutrition? This could be a biggy for some patients. One size fits all may be difficult for some patients?

Does the Dr have a nutritionist on staff and is there a cost?(after surgery a nutritionist could be the difference between success and failure)

Is there a sychotherapist who specializes in eating disorders available should you need one?

I assume you've already chosen your Dr and your satisfied he/she has 2+ years experience, >250 bands placed, practiced medicine in your area for >2 years?

BTW-Are you sure you have to have the 6 months supervised nutrition program? BCBS required this for many years and have eliminated this requirement last year.

I do have to have the 6 months because I have Medicare :( I really wish they would eliminate it as well. Thanks for the questions, you have been a big help!!!
