Pre-Cert Question

I just found out that my insurance will cover most of the cost of Lap Band but they said I need to have prior authorization from a doctor. What kind of appointment would I make to do this? Do I just go see a regular doctor? Any help/information you have would be great, I want to get this movin!! :lol: Thanks!!
