How do you know when you need a fill? Is this typically covered by insurance?

Mine is covered because my surgery was covered . I got it a five weeks.If you are not losing or you are hungry more often.He ask me to eat a lean Q and tell me how much I could eat. I could eat all and 1/2 another at least.
